: Ethics approval for this observational study was obtained from the Ethics Commission of the Medical Department of Goethe University, Frankfurt am Main, Germany (No. 122/09). The study was performed in compliance with the ethical principles of good clinical practice and according to the International Conference on Harmonisation Harmonised Tripartite Guideline for Good Clinical Practice.
